LCrRLJ 4.2.  PROCEDURE UPON A PLEA OF GUILTY

     (i)  Guilty Plea Statement.

     It shall be the duty of the Defense Attorney to have a
     properly completed written statement of the defendant on a
     guilty plea.  Forms shall be furnished by the Court
     Administrator without charge.
