Rule 3
                               Form of Pleading


Every paper presented to the Judge for signature or delivered to the Court
Clerk for filing shall be a clearly readable original and shall include:

   (a) number and title of case;

   (b) designation of what the paper purports to be;

   (c) name, original signature, office address, telephone number and Washington
       State Bar Association Number of counsel.
