JURY TRIALS
(d) Impaneling the Jury
(1) Voir Dire
(A) Completion of Voir Dire - Juror Questionnaires
and Information Form. At the conclusion of voir
dire, Juror Questionnaires, if any, and Information
Forms shall be immediately returned to the Court.
Juror Questionnaires and Information Forms may not
be removed from the courtroom without the express
permission of the trial Judge. Juror Information
Forms and Questionnaires will not be filed with the
Court file. The trial Judge shall direct that
individual Questionnaires of jurors dealing with
matters of a personal or sensitive nature, if
required to be filed by rule or statute, shall be
separately filed under seal.

(3) Peremptory Challenges
(A) The exercise or waiver of peremptory
challenges shall be noted secretly on the jury list.
