      SERVICE AND FILING OF PLEADINGS AND OTHER PAPERS


     (d) Filing.

     (5) Motions.  No motion for any order shall be heard
unless the papers pertaining to it have been filed with the clerk.

     (6) Documents Not to Be Filed.  Unanswered
interrogatories to parties and requests for admissions where
an answer or other response is expected on the same document
shall not be filed unless necessary for the disposition of a
motion or objection.  Photocopies of reported cases,
statutes or texts shall not be filed as an appendix to a
brief or otherwise but may be furnished directly to the
judge hearing the matter.  Documents or copies thereof
produced during discovery and other items which should
properly be received as exhibits rather than as a part of
the court file shall not be included in the court file.

     (7) Indexing Cover Sheet.  An indexing cover sheet
(Clerk Form 1) shall be completed and filed with all initial
pleadings at the time the pleadings are assigned a cause number.
