  FILING AND SERVICE OF PROCESS OF DEPENDENCY, GUARDIANSHIP
                  AND TERMINATION PETITIONS


     The petitioner shall provide the Clerk of the Court
with the original petition along with sufficient copies of
the petition for service of process.  The party filing a
petition shall also file a Request for Service notifying the
Clerk of Court what type of service is being requested.  The
Clerk of Court shall notify the petitioner when personal
service packets are completed.  The petitioner is
responsible for completing personal service and filing a
declaration of service.  The Clerk is responsible for
service by publication and mail and for filing a declaration of service.
