On the cover page, below “Effective Dates,” change the “Instructions” effective date to July 22, 2007 and change the “Worksheets” effective date to July 22, 2007.
On Page 1 of the “Washington State Child Support Schedule Instructions for Worksheets,” insert the following at the top of the left column:
“Child Support Order Summary Report:
Fill out the Child Support Order Summary Report only if you are also submitting a temporary or a final child support order to be signed by a judge, commissioner or administrative law judge.”

However, both parents have an obligation to pay their share of extraordinary health care expenses. Extraordinary health care expenses mean those monthly medical expenses that exceed 5% of the basic support obligation from the Child Support Schedule Worksheet, Line 5.
The father shall pay __________% of extraordinary health care expenses (unless stated otherwise, the father’s proportional share of income from the Worksheet, line 6) and the mother shall pay ___________% of extraordinary health care expenses (unless stated otherwise, the mother’s proportional share of income from the Worksheet, line 6).”
