SUBJECT: 2007 SUMMARY OF CHANGES TO MISDEMEANOR JUDGMENT AND SENTENCING FORMS

The Pattern Forms Committee approved the following changes to the Misdemeanor Judgment and Sentencing Forms:

I.	CrRLJ 04.0200, “Offender Registration” Attachment In paragraph 5, delete “(Effective September 1, 2006)” and “If I am enrolled on September 1, 2006, I must notify the sheriff immediately.”
2.	CrRLJ 07.0900, Domestic Violence No-Contact Order On page 2, under “It is Further Ordered,” insert the following as the second check box option: “☐ The appropriate law enforcement agency shall at a reasonable time and for a reasonable duration assist the defendant in obtaining personal belongings located at: ____________________________________________________________.”
