In RE the Matter of Court Administration Order RE: Filing Proof of Service by Mail

FILING PROOF OF SERVICE BY MAIL )  

 

When a person filing a paper with the Court of Appeals serves it by mail, RAP 18.5(b) states that service should be as provided in CR 5(b).

CR 5(b)(2) requires that papers served by mail must be addressed to the person on whom they are being served. Proof of service by mail may be by written acknowledgment of service, by affidavit of the person who mailed the papers, or by certificate of an attorney.

To assist the court in ascertaining if a paper has been properly served by mail,

IT IS ORDERED:

All affidavits and certificates of mailing filed with this court shall include the date of service, the name and title of the person served and the address where the papers were mailed. The certificate of an attorney shall be in form substantially as set forth in CR 5(b)(2)(B) and unsworn statements in lieu of affidavits shall be in form substantially as set forth in GR 13.
