SUMMARY OF CHANGES TO DOMESTIC VIOLENCE FORMS
Use this sheet to track changes made in Fall 2000 to the forms.
All of the forms were modified to comply with GR 14 margin requirements, except for the Law Enforcement Information Sheet and the Foreign Protection Order Information, which are law enforcement documents. You may use the revised forms now and must use them by April 1, 2001, the GR 14 implementation date.
