Summary of Changes to Misdemeanor Judgment and Sentencing Forms

Use this sheet to track changes made in Fall 2000 to the forms.

All of the forms were modified to comply with GR 14 margin requirements. You may use the revised forms now and must use them by April 1, 2001, the GR 14 implementation date.

CrRLJ 03.0400

Rights, Conditions and Warnings

In the footer, "(DUI)" was inserted after the document title.

CrRLJ 07.0100

DUI Judgment and Sentence Form

To comply with GR 14, the last line above the date line was changed to: "I have read the rights, conditions and warnings." The phrase "on the back of this form" was removed from the end of the sentence. Pursuant to GR 14, "…[t]he writing or printing shall appear on only one side of the page…"

CrRLJ 07.0110

Judgment and Sentence Form

On the second page, the line regarding the order's expiration date was changed to "THIS NO-CONTACT ORDER EXPIRES ONE YEAR FROM TODAY ON _____________________________."

Harassment No-Contact Order

The line regarding the order's expiration date was changed to "THIS NO-CONTACT ORDER EXPIRES ONE YEAR FROM TODAY

ON _________________________________."
