In the bottom right corner of the form, change "Respondent's History Includes" to "Restrained Person's History Includes."

Petition For An Order For Protection

Delete paragraphs 4, Identification of Petitioner, and 5, Identification of Respondent.

Add the following as the last restraint request:

    REQUIRE the respondent to pay the fees and costs of this action.

Insert the following prompt within the Statement:

    If you requested a fee waiver, describe the incident(s) involving stalking, a sex offense, or domestic violence:

Change the language used to request petitioner's address for legal service as follows:

    You have a right to keep your residential address confidential. You may list an address that is not your residential address where you agree to accept legal documents:_______________________

Notice of HearingThis is a new form which may be used to schedule hearings on motions and on the petition if an ex parte temporary order for protection is not issued.

Return of ServiceIn the list of served documents, in the fourth check box option in the left column, delete "from Civil" from the form name which is now "Order for Protection - Harassment."

In the right column, change the first check box option to "Motion to Modify/Terminate Order for Protection." Delete the second check box option and add two lines in the "Other" check box option.

Order for Protection - Harassment

Change the form title from "Order for Protection from Unlawful Civil Harassment" to "Order for Protection - Harassment."

Add "DOB" for petitioner and respondent in the caption.

Some antiharassment protection orders provide protection from a person who has committed a violent or threatening act. Possible instances include stalking, a sex offense or domestic violence. In addition to those three situations specifically mentioned in Chapter 10.14 RCW pursuant to Chapter 117 L 2002 amendments, other situations may include 2nd degree assaults. The protected persons in those cases are eligible for full faith and credit protection. The following changes based upon 18 U.S.C. §§ 2265 and 2266(5).

Add the following optional paragraph 1:

    1. Full Faith and Credit: This order is issued to prevent violent or threatening acts of harassment. The court has jurisdiction over the parties, the minors and the subject matter. This order is issued in accordance with the Full Faith and Credit provisions of VAWA. 18 U.S.C. § 2265.

Insert the following new restraint above the option "Other:"

    Judgment is granted against respondent for fees and costs in the amount of $______.

The former form UH-09.0100, Motion to Modify Order for Protection and Notice of Hearing is deleted and replaced with this new motion for modifying or terminating a temporary or full order for protection.

Order Modifying/Terminating Order for Protection - Harassment

The former form UH-09.0200, Order Modifying Order for Protection, is deleted and replaced with the new order modifying or terminating a temporary or full order for protection.

Motion to Terminate Order for Protection and Notice of Hearing

This form is deleted.

Order Terminating Order for Protection - Harassment

This form is deleted.

DV-6.020

Denial/Dismissal Order

This is a new form which may be used to deny or dismiss various petitions or motions. This form may be used in domestic violence and in antiharassment cases.
