This form is based upon RCW 26.27.441(1) which authorizes the request for registration of foreign child custody determinations.

Motion/Declaration for Ex Parte Order Allowing Petitioner to Arrange Service (Optional Use)

Pursuant to RCW 26.27.441(2), the court is required to serve notice of the request to register a Child Custody Determination on the persons named in the request for registration. The Motion/Declaration for Ex Parte Order Allowing Petitioner to Arrange Service is for optional use in counties that do not have a procedure for the court to accomplish the service.

This form is based upon RCW 26.27.471(1) and (2), which authorize the petition for enforcement and list the requirements of the petition.

This form is based upon RCW 26.27.471(3) and (4), which authorize the court to order the respondent to appear at a hearing with or without the child on the next judicial day after service or the next judicial day possible and list required notices to the respondent.

Order on Petition for Enforcement of Child Custody Determination Under UCCJEA

This form is based upon RCW 26.27.491 and .511, and is used after the hearing on the Petition for Enforcement of Child Custody Determination and Issuance of Order to Show Cause Under UCCJEA.

Confidential Information Form

This form is used for a combination of mandatory data collection requirements. This form is required pursuant to RCW 26.09.020(3), 26.21.305(3), 26.23.050(7) and 26.23.055. It is used to collect information for data entry into the Judicial Information System pursuant to RCW 26.50.160. It is also used to collect personal information, such as a residential address, required under the UCCJEA, RCW 26.27.281.
