SUBJECT:MISDEMEANOR JUDGMENT AND SENTENCING FORMS – June, 2008, UPDATE
 

Please find enclosed an updated set of Misdemeanor Judgment and Sentencing forms.  Please see the Summary of Changes for detailed information regarding the changes.  These forms are suggested, not mandatory.  You are encouraged to adapt the forms to local practice.  All forms may be freely reproduced.

The margins of the enclosed forms reflect the following approach to compliance with GR 14 margin requirements:  If a form will be date stamped by the clerk before it is filed, the form will have a three inch top margin on the first page.  If a form will be filed by the court without a date stamp, the form will have a one inch top margin on the first page.

To obtain additional copies of the forms, you may download the forms from the Washington State Court’s Web site at: http://www.courts.wa.gov/forms.  You may also order the forms on CD or on paper by using the enclosed order form.
