CAUSE-OF-ACTION REPORTING CATEGORIES

PARENTAGE/URESA/UIFSA

Parentage/URESA/UIFSA (PUR) - Petition to determine the legal status of a parent which is filed in conjunction with the reciprocal report entered under the URESA or UIFSA acts. A URESA case is always an originating action, and should be recorded as a new filing.

PARENTAGE

PARENTAGE (PAT) - Petition to determine the legal status of a parent.

MODIFICATION

Modification (MOD) - Change of a previous order or decree.

    Note: Use only when a new case file is opened and a new case number assigned. This will occur when the decree or order at issue was previously entered in another county or state.

RELINQUISHMENT

Relinquishment (REL) - A petition to voluntarily relinquish parental rights of a child to the DSHS, an agency, or a prospective adoptive parent. If the petition involves adoption within the county, use the "ADP" (adoption) cause code (RCW 26.33.090).

TERMINATION

Termination of Parent/Child Relationship (TER) - A petition for a termination of a parent-child relationship of a parent or alleged parent who has not executed a written consent. If the petition involves adoption within the county, use the "ADP" (adoption) cause code (RCW 26.33.100).

ADOPTION

Petition to establish new parent-child relationship(s), or adoption-related petition not addressed by other causes of action.

Adoption (ADP) - A petition for the establishment of a new, permanent relationship of parent and child between persons not having that relationship. This can also include cases involving the termination of parent-child relationships (RCW 26.33.150).

Miscellaneous (MSC) - Any other petition not specified by other codes (e.g., confidential intermediary).

CONFIDENTIAL NAME CHANGE

Confidential Name Change (CHN) - Petition for a confidential change of name for a victim of domestic violence (RCW 26.50.010).

INITIAL PRE-PLACEMENT REPORT FILED WITH CLERK

Initial Pre-Placement Report Filed With Clerk (PPR) - An initial pre-placement report filed on a child by the DSHS prior to the filing of adoption papers (RCW 26.33.190). Subsequent pre-placement reports filed for an existing case should be filed with the initial report, and are not counted on the monthly caseload report form. If an initial pre-placement report is filed after an adoption case is opened, include the pre-placement report with the adoption case. These matters are normally resolved and completed at the time they are opened
