Domestic Relations Plain Language Forms

The Access to Justice Board's Pro Se Project Forms Review Work Group is converting the mandatory domestic relations pattern forms into plain language. The workgroup's goal is to create forms that allow parties to understand the forms and the legal concepts easily and completely. To help achieve that goal, please review the following plain language forms and submit comments about the forms or the Project using the comment form to the right. The comment form allows you to type comments and to upload a form that you filled out.
