Guide to Filing Complaint (Grievance)
Regarding the Conduct of a Certified Professional Guardian

Please choose a convenient method to file your complaint. If you have supporting documents that are not in electronic format, you may mail hard copies to the address below (see U.S. Mail). Please include a copy of the completed complaint form.

You may email the completed complaint form you downloaded and supporting documents to guardiangrievances@courts.wa.gov.

FAX:
You may fax the completed complaint form you downloaded to 360-956-5700 (Attn: Certified Professional Guardian Board). Be sure to include all required information to prevent delay in processing your complaint.
